Privacy Policy Statement

Website tools overview — updated October 15, 2026

This section explains the forms and website measurement tools currently linked from this website. The agency privacy and insurance-information notices follow below.

Which form am I using?

  • Short inquiries — Google Forms: Our agency-owned form asks for your name, email OR phone, state, selected coverage and an optional brief question. Responses are stored in the agency Google Forms account, and the owner receives email notifications. Google sign-in is not required. We use this information to review and respond to your inquiry.
  • Detailed applications — RiskAdvisor / QuoteForms: These forms ask additional applicant and coverage questions. Information you submit is handled through RiskAdvisor for the agency’s review.
  • Policy service and contact — Secure Form Solutions: These forms handle requests such as ID cards, certificates, policy changes and general messages. Review the notices on the form before submitting.
  • Medicare review — RetireFlo: Our linked Medicare questionnaire uses RetireFlo. Review its notices and requested information before proceeding; completing it does not enroll you in a plan.
  • Payments and claims: Links can open insurer websites or our agency payment provider, AndDone. Those providers handle information entered on their pages. Check the destination and its privacy notice before entering payment or policy details.

Do not put Social Security numbers, Medicare numbers, medical records, prescription lists, payment details or identity documents into short inquiries, general comments or ordinary email. Ask our team how to provide any documents required for the next step.

Website measurement

We use Google Analytics 4 to understand page visits and website actions such as phone-link clicks, request starts, form handoffs and confirmed submissions where supported. Our custom website events include the coverage category, form provider and action type; they do not read your name, contact fields or message. A click or handoff is not proof that a request was completed.

When you open our short inquiry from this website, we may add an optional random website reference. Our agency Google Apps Script uses that reference to connect the website’s Google Analytics browser and session identifiers with a confirmed Google Form submission. The reference is not your name or contact information. You can remove it from the form without affecting your request.

The submission event sends Google Analytics the selected website coverage category, form provider, action type and existing analytics identifiers. The integration does not send your name, email, phone number, state answer, message or other form answers to Analytics. The matching record expires after one hour and is removed by scheduled hourly cleanup, subject to Google scheduling delays. After a reference is used, its Analytics identifiers are removed from the matching record. A receipt containing a non-contact hash and processing status is retained for seven days to help prevent duplicate counts, then removed by scheduled cleanup. The inquiry and its optional reference remain in the agency Google Forms account under the form’s normal retention.

Analytics blocking, missing or expired references, and inquiries opened directly in Google Forms may prevent matching a completion to a website visit. The agency’s Google Forms response records remain the source for total submissions. A completed inquiry is a submitted request, not a quoted or issued policy.

Google Analytics may use cookies and collect browser, device and usage information under Google’s practices. See Google’s privacy policy. Embedded services, translation tools and external providers may also process technical information when used.

Questions about submitted information

Call our agency at (800) 310-2743 or use our contact page to ask about a response, correction or privacy concern. Identify the form and approximate submission date without repeating sensitive information.

Personal information about you, including information from a credit report and loss history may be collected from persons other than you in connection with this application for insurance and subsequent policy renewals. You have the right to review your personal information in our files and can request correction of any inaccuracies. Such information as well as other personal and privileged information collected by us or our agents may in certain circumstances be disclosed to third parties without your authorization to assist in servicing your account. Any person who knowingly and with intent to defraud any insurance company or another person files an application for insurance or statement of claim containing any materially false information, or conceals for the purpose of misleading information concerning any fact material thereto, commits a fraudulent insurance act, which is a crime and subjects the person to criminal and (NY: substantial) civil penalties. (Not applicable in CO, HI, NE, OH, OK, OR, or VT; in DC, LA, ME, TN, VA and WA, insurance benefits may also be denied).

Cookies and browser storage
This website and its third-party tools may use cookies and browser storage for functionality and measurement. Our request measurement uses session storage to associate a recent handoff with a supported confirmation return. The stored handoff contains the request category, provider and time, rather than your form answers. Cookie and storage lifetimes vary by tool; they are not all limited to a 20-minute visit. You can manage cookies and site data through your browser settings. Blocking storage or third-party services may affect forms, translation or measurement.

Third Party Links
This Web site may contain links to third party Web sites that are not controlled by Direct Insurance Network. These third party links are made available to you as a convenience and you agree to use these links at your own risk. Please be aware that we are not responsible for the content of third party Web sites linked to us nor are we responsible for the privacy policy or practices of third party Web sites linked to Direct Insurance Network. Our Privacy Policy only applies to information we collect from you while you are at and while you are actively in a session with Direct Insurance Network. If you should link to a third party Web site from Direct Insurance Network, we strongly encourage you to review and become familiar with that Web site's privacy policy.

Changes In Our Privacy Policy
We reserve the right to change this Privacy Policy without providing you with advance notice of our intent to make the changes.

If you have any questions about our privacy policy, please feel free to contact us at (800) 310-2743.
